Talk to your insurance provider

You can submit a predetermination request to your insurance provider before purchasing EpiMonitor to find out if it may be reimbursed. This optional step can give you a clearer understanding of your potential coverage.


We provide a predetermination letter template that your doctor can personalize and fill out on your behalf. You can send this directly to your insurance agent.

We recommend using HCPCS Code E1399
While Empatica is not a Durable Medical Equipment (DME) Supplier, some of our users have successfully received reimbursement using HCPCS Code E1399: Durable medical equipment, miscellaneous. Because E1399 is a miscellaneous code, coverage varies by plan and insurer.

EpiMonitor is a prescription medical device, not a medical service, and Empatica is not a clinical provider. It is not associated with a CPT code or NPI number.

Can Empatica bill my insurance provider directly?

Empatica is not currently registered as a DME supplier, so we are unable to bill insurance providers directly. However, some of our users have successfully submitted reimbursement requests using their EpiMonitor purchase receipt and documentation we provide. We encourage you to check with your insurance provider and provide them with detailed support materials.

How much does EpiMonitor cost out of pocket?

EpiMonitor costs $399 for a one-off purchase of the EmbracePlus wearable, plus a monthly fee for a subscription. There are three subscription plans available, with the cheapest costing $15.90 per month. You can see all subscription options here and choose the one that best suits your needs. But rest assured, all subscription options include seizure alerting. All orders come with a 30-day trial period, so if you change your mind you can return your device to us and receive a full refund.

What is included in the Standard subscription plan?

The Standard plan is our most popular subscription plan. It costs $24.90 per month and provides automated seizure alerts, including an SMS with the user’s location, to up to 3 caregivers. It also includes a seizure diary, plus sleep and activity stats for the previous 7 days.

How can someone order and receive EpiMonitor?

Empatica handles all orders through its online store, where someone can select their wearable color and subscription plan. Once a purchase is made and a valid prescription is uploaded to the user's account, the EpiMonitor pack will be shipped to their address, including one EmbracePlus device and instructions on how to download the companion EpiMonitor app.

How can a doctor prescribe EpiMonitor and where do they send the prescription?

To prescribe EpiMonitor for their patients, doctors need to provide them with a paper or PDF prescription containing the information outlined here. The patients will then upload an image of this to the Empatica system after purchasing EpiMonitor, so a member of the Empatica team can review its validity and release the shipment. If the prescription is not valid, Empatica will contact the patient directly to explain why. If the patient is not able to provide a valid prescription to Empatica, their order will be cancelled and fully refunded.

I already have an EpiMonitor prescription, where do I need to send it?

You can upload your EpiMonitor prescription to your account, which you'll be asked to create after your purchase. Since EpiMonitor is FDA-cleared, we will need a valid prescription to ship your order. After completing your purchase, you can directly share your prescription copy alongside your purchase invoice with your insurance provider to request reimbursement. Please note that Empatica does not send prescriptions to insurance providers.

How can EpiMonitor support someone with their epilepsy monitoring?
